I can suggest two ways to do it.  First, have a Wikipedia-style, but automatic word recognition cross-reference linking for less common words that you can also add to the end in an index (Note that if this is a potential option, consider letting the reader have the ability to turn that linking on and off so that the text is not cluttered).

Second option is just to let annotators do limited manual linking within the annotations.
